Delta’s collection charge shall be reduced to $5.00. 5. Delta’s reconnection charge shall be reduced to $9.00. 6. Delta’s Light Pilot Charge is approved at $35.00 per hour, with a minimum charge of one hour, and shall be included in Delta’s tariff. 7. Delta shall include the following language in the Application for Service section of its tariff: “To confirm the identity of prospective customers, Delta requires the following information: Name; Complete Social Security Number, last four digits of Social Security Number, or Driver’s License Number. In addition, when service is turned on, the prospective customer will be required to present photo identification, or an alternate form of identification for someone who does not have photo identification, confirming his or her identity.” |
