Final Order Entered: 1. T.L. Maddux; Alan Hunt; Glenn Hughes; Tony Travis; Darrell Simpson; and Tim Capps; and its manager Addie Adamson are each assessed a civil penalty of $100 to be paid within two years of the service of this Order for their willful violations of KRS 278.160. 2. Payment pursuant to ordering paragraph 1 shall be made by cashier’s check or money order payable to the Kentucky State Treasurer and mailed or delivered to the Office of the General Counsel, Kentucky Public Service Commission, 211 Sower Boulevard, Frankfort, Kentucky 40601. 3. The Commission shall waive the civil penalty of $100 for each individual, T.L. Maddux; Alan Hunt; Glenn Hughes; Tony Travis; Darrell Simpson; and Tim Capps; and its manager Addie Adamson who attends a total of 12 hours of Commission-approved training within 365 days of this Order. The commissioners and manager shall provide written notice of their presence at the training within 30 days of attendance; and follow all other requirements contained in this Order. 4. Crittenden-Livingston District shall file policies and procedures on review of Commission Orders and submission of water purchase agreements in post-case correspondence within 90 days of the Order. 5. Within 20 days of this Order, Crittenden-Livingston District shall file the March 26, 2021 updated purchased water contract and any other outstanding contracts through the tariff filing for Commission review.

Order Entered: 1. The procedural schedule set forth in Appendix A to this Order shall be followed. 2. On or before the date set forth in the procedural schedule, Crittenden-Livingston District shall file its responses to Commission Staff’s request for information, attached to this Order as Appendix B. 3. Crittenden-Livingston District shall respond to all requests for information propounded by Commission Staff, whether identified on the procedural schedule or otherwise, as provided in those requests. 4. Any party filing a paper with the Commission shall file an electronic copy in accordance with the electronic filing procedures set forth in 807 KAR 5:001, Section 8. Electronic documents shall be in portable document format (PDF), shall be searchable, and shall be appropriately bookmarked. The Commission directs the parties to the Commission’s July 22, 2021 Order in Case No. 2020-00085 regarding filings with the Commission.

Order Entered: 1. This case is opened to investigate the alleged failure of Crittenden-Livingston District to comply with KRS 278.160 and any applicable Commission Order, including its November 4, 2025 Order as referenced herein. 2. Crittenden-Livingston District, each of its commissioners, and the general manager, in their official and individual capacities as officers of Crittenden-Livingston District’s Board of Commissioners, shall submit individual, written responses to the allegations contained in this Order within 30 days of the date of service of this Order, including but not limited to any, and all written testimony or documentation that would dispute the allegations. 3. Each response shall include the name of the witness responsible for responding to the questions related to the information provided. 4. Each response shall be answered under oath or, for representatives of a public or private corporation or a partnership or association or a governmental agency, be accompanied by a signed certification of the preparer or the person supervising the preparation of the response on behalf of the entity that the response is true and accurate to the best of that person’s knowledge, information, and belief formed after a reasonable inquiry. 5. Counsel for each party shall enter an appearance or appearances within 20 days of the date of service of this Order.
