Sage Telecom Communications Amendment to Lifeline Plans

Order Entered: 1. The designation of Sage as a limited ETC is conditioned on Sage's compliance with: (1) the representations and commitments made by Sage in its ETC application and its Compliance Plan; and (2) the FCC rules, including those adopted by the FCC in the Lifeline Reform Order. 2. Sage is designated as an ETC for the purpose of offering Lifeline service only in the underlying carrier's licensed service area in Kentucky. 3. During the current certification period, Sage shall be eligible to receive federal USF support for Lifeline. Sage shall also be eligible to receive Kentucky USF support for Lifeline subject to the submission of and approval by the Commission for a Kentucky specific Lifeline plan for Kentucky subscribers which provides benefits commensurate with the amount of Kentucky USF support. 4. Sage shall advertise the availability of and charges for these services using media of general distribution. 5. Sage shall comply with the FCC's annual certification process for Lifeline customers. 6. Sage is a utility under the definitions contained in KRS 278.010(3), and shall include revenue generated from the sale of intrastate wireless service, including Lifeline revenues, in its reports filed pursuant to KRS 278.140. 7. Sage shall file its specific Lifeline plan for Kentucky subscribers within 20 days of the date of this Order. 8. This case shall remain open until further Commissioner Order.
