Final Order Entered: 1. The purchased water adjustment factor of $0.15 per 1,000 gallons is approved. 2. The rates proposed by Breathitt District are denied. 3. The rates as set forth in Appendix B to this Order are approved for water service rendered by Breathitt District on and after July 1, 2024. 4. The Commission hereby amends final Order in Case No. 2022-00136 to the extent that Breathitt District shall use the rates set out in Appendix B of this Order to issue the requisite customer notice on or before the implementation of the Phase 2 and Phase 3 rates set forth in the final Order in Case No. 2022-00136. Any remaining provisions of the final Order in Case No. 2022-00136 not inconsistent with this Order shall remain in full force and effect. 5. Within 20 days of the date of service of this Order, Breathitt District shall file with the Commission, using the Commission’s electronic Tariff Filing System, revised tariff sheets showing the rates approved herein. 6. On or before May 31, 2025, Breathitt District shall file a revenue sufficiency report based on audited 2024 financial statements. 7. This case is closed and removed from the Commission’s docket.
