Verizon Wireless shall transmit new letters by certified mail, return receipt requested, to Larry Joe Watt, Davis Keith, John W. and Lori Ann Meredith, Todd K. and Lisa Michelle Webb, Steven and Kimberly Bowles, Tara L. Murphy, T J and Alex Massey, and Julie Skaggs Reed, providing notice of this proceeding in accordance with 807 KAR 5:063, Section 1(1)(l). 2. Order Entered: 1. This case is reopened to investigate whether the record landowners of property within 500 feet of Verizon Wireless’s proposed construction site received notice of Verizon Wireless’s application compliant with KRS 278.665(2) and 807 KAR 5:063, Section 1(1)(l). 2. The procedural schedule set forth in the Appendix A to this Order shall be followed. 3. Verizon Wireless shall file a response to the request for information attached to this Order as Appendix B on or before February 24, 2023. 4. Verizon Wireless shall respond to all requests for information propounded by Commission Staff, whether identified on the procedural schedule or otherwise, as provided in those requests. 5. Nothing contained herein shall prevent the Commission from entering further Orders in this matter.

Final Order Entered: 1. Verizon Wireless is granted a CPCN to construct a wireless telecommunications facility. The proposed facility consists of a tower not to exceed 199 feet in height, with attached antennas, to be located near 4229 Otter Gap Road, Bowling Green, Kentucky 42101, in Edmonson County, Kentucky. The coordinates for the proposed facility are North Latitude 37° 05' 02.74" by West Longitude 86° 16' 10.91". 2. Verizon Wireless shall immediately notify the Commission in writing if, after the antenna tower is built and utility service is commenced, the tower is not used for three months in the manner authorized by this Order. 3. Verizon Wireless shall file a copy of the final decision regarding the pending KAZC application for the proposed construction within ten days of receiving a decision. 4. Documents filed, if any, in the future pursuant to ordering paragraphs 2 and 3 herein shall reference this case number and shall be retained in the post-case correspondence file. 5. This case is closed and removed from the Commission’s docket.
