Final Order Entered: 1. Center Ridge is assessed a penalty of $2,500 for its willful failure to comply 2. Center Ridge shall pay the assessed penalty, totaling $2,500.00 within 30 days of the date of entry of this Order. Payment shall be by certified check or money order made payable to "Treasurer, Commonwealth of Kentucky" and shall be sent by certified mail or delivered to Office of General Counsel, Public Service Commission of Kentucky, 211 Sower Boulevard, P.O. Box 615 Frankfort, Kentucky 40602-0615. 3. The Executive Director shall serve a copy of this Order on Center Ridge by certified mail. |

Order Entered: 1. Center Ridge shall appear on January 9, 2018, at 9:00 a.m. Eastern Standard Time, in Hearing Room 1 of the Commission's offices at 21 1 Sower Boulevard, Frankfort, Kentucky, to show cause, if any, why it should not be subject to penalties pursuant to KRS 278.990 for failure to satisfy the matters complained of or file a written answer to the complaint in Case No. 2017-00164, as required by 807 KAR 5:001, Section 20(6), and the Commission's Order entered April 18, 2017, in that case. 2. Pursuant to KRS 278.360 and 807 KAR 5:001, Section 9(a}a, the record of the formal hearing shall be recorded by digital video recording only. 3. The record of Commission Case No. 2017-00164 is made a part of the record by reference only. 4. Any request for an informal conference with Commission Staff to discuss the issues in this case shall be set forth in writing and filed with the Commission within 20 days of the date of this Order. |
