Order Entered: Warren District is authorized to assess the proposed Buchanon Park SICRA fee, as set forth in Appendix A to this Order, for each connection made to the Buchanon Park sewer main extension on and after the date of this Order. 2. Warren District shall cease assessment of the Buchanon Park SICRA fee on December 31, 2031 or when its cumulative assessment of Buchanon Park SICRA fees totals $1,122,012 or when it has paid the KIA loan which it executed to finance the Buchanon Park sewer main extension, whichever occurs first. 3. Within 30 days of the date of this Order, Warren District shall file revised tariff sheets that reflect the fees approved in this Order and contain a description of the area that is subject to the Buchanon Park SICRA fee. At a minimum, this description shall include a map of the Buchanon Park SICRA and a listing of all streets and roadways within the Buchanon Park SICRA. 4. Warren District shall maintain a current description of the Buchanon Park SICRA in its filed rate schedules as long as the Buchanon Park SICRA fee remains in effect. 5. Beginning in 2013, for the period from the date of this Order through December 31, 201 2, and for each calendar year thereafter in which the surcharge is effective, Warren District shall submit with the annual financial and statistical report required by 807 KAR 5:006, Section 3(1), a written report stating the total Buchanon Park SiCRA fees billed during the preceding calendar year and the total Buchanon

Order Entered: 1) Warren District is granted a CPC&N. 2) Warren District shall notify Commission prior to performing any additional construction. 3) Any deviation shall be undertaken with prior approval of Commission. 4) Warren District shall furnish documentation of total costs of project within 60 days of substantial completion. 5) Warren District shall file copy of "as-built" drawings and a certified statement that the construction has been satisfactorily completed in accordance with the contract plans and specifications within 60 days of substantial completion. 6) Warren District shall require construction be inspected under general supervision of a licensed professional engineer with a Kentucky registration in civil, electrical, or mechanical engineering. 7) Warren District shall notify Commission 7 days prior to start of construction and at 50% completion point. 8) Warren District is authorized to enter into loan agreement with KIA for amount not to exceed $823,900 with term of 20 yrs. at interest of 3% per annum. 9) Within 30 days of executing proposed loan agreement with KIA, Warren District shall advise Commission of execution and of any terms that differ. 10) Warren District shall use proceeds from loan agreement with KIA for purposes set forth in application. 11) The operation of the proposed Buchanon Park Sewer Interceptor Capital Recovery Area Contribution Fee is suspended for a period of five months, from January 1, 2012 up to and including May 31, 2012.
