Order Entered: 1. The water service rates and the 5/8- x 3/4-inch meter tap-on fee requested by Farmdale District are denied. 2. The water service rates and nonrecurring charges set forth in Appendix A to this Order are approved for services rendered by Farmdale District on and after the date of this Order. 3. Within 20 days of the date of this Order, Farmdale District shall file revised tariff sheets with the Commission, using the Commission's electronic Tariff Filing System, setting forth the rates approved herein. 4. In all future reporting periods, Farmdale District shall use the "PSC Approved Service Life" set forth in Appendix B to this Order for the listed asset groups when calculating and reporting depreciation for all reporting periods after the date of this Order. No retroactive adjustment to accumulated depreciation or retained earnings to account for this change in accounting estimate shall be made. 5. The Executive Director is delegated authority to grant reasonable extensions of time for the filing of any documents required by this Order upon Farmdale District's showing of good cause for such extension.

Order Entered: 1. Farmdale District's motion for an extension of time to respond to the Initial Requests for Information is granted. 2. Farmdale District shall file responses to the questions asked in the attached Appendix to the Commission's March 17, 2014 Order no later than May 1, 2014. 3. No later than June 30, 2014, Commission Staff shall file with the Commission and serve upon all parties of record a written report containing its findings and recommendations regarding Farmdale District's requested rate adjustment. 4. The parties shall file with the Commission their written comments on and any objections to the findings and recommendations contained in the Commission Staff Report no later than July 14, 2014, or 14 days after the date of the filing of the Commission Staff Report, whichever occurs earlier. Any party may file with such submission any additional evidence for the Commission's consideration. 5. A party's failure to file written objections to a finding or recommendation contained in the Commission Staff Report by July 14, 2014, or 14 days after the date of the filing of the Commission Staff Report, whichever occurs earlier, shall be deemed as agreement with that finding or recommendation.

Order Entered: 1. a. Farmdale District shall file responses to the questions asked in the attached Appendix within 14 days of the date of this Order. Responses to requests for information shall be appropriately bound, tabbed and indexed and shall include the name of the witness responsible for responding to the questions related to the information provided, with copies to all parties of record and five copies to the Commission. b. Each response shall be under oath or, for representatives of a public or private corporation or a partnership or association or a governmental agency, be accompanied by a signed certification of the preparer or person supervising the preparation of the response on behalf of the entity that the response is true and accurate to the best of that person's knowledge, information, and belief formed after a reasonable inquiry. c. A party shall make timely amendment to any prior response if it obtains information which indicates that the response was incorrect when made or, though correct when made, is now incorrect in any material respect. d. For any request to which a party fails to furnish all or part of the requested information, that party shall provide a written explanation of the specific grounds for its failure to completely and precisely respond. e. The party responding to a request for information shall serve an original and a copy on the Commission and serve a copy upon all parties of record.
