Order Entered: 1. Except where they contradict or conflict with the findings contained in this Order, the findings contained in the Commission Staffs report are adopted and incorporated by reference into this Order as if fully set out herein. 2. The rates set forth in Appendix C are approved for water service that Crittenden-Livingston District renders on and after January 1, 2013. 3. The rates set forth in Appendix D are approved for water service that Crittenden-Livingston District renders on and after March 1, 2014. 4. The rates set forth in Appendix E are approved for water service Crittenden-Livingston District renders on and after May 1 , 2015. 5. Between February 1, 2014 and February 15, 2014, Crittenden-Livingston District shall notify its customers of the implementation of Phase II rates. 6. Between April 1, 2015 and April 15, 2015, Crittenden-Livingston District shall notify its customers of the implementation of Phase Ill rates. 7. Within 20 days of the date of this Order, Crittenden-Livingston District shall file the revised tariff sheets with the Commission. 8. Crittenden-Livingston District shall for use the “Newly Assigned Service Lives” set forth in Appendix B for the listed classes of assets when calculating and reporting depreciation for all reporting periods after the date of this Order, but shall make no retroactive adjustment to its books to account for the cumulative effect of this change in accounting estimate.

Order Entered: 1. This proceeding is established to review the reasonableness of Crittenden-Livingston’s proposed rates.’ 2. No later than 10/31/12, Commission Staff shall file and serve all parties of record a written report containing its findings and recommendations. 3. The parties shall file written comments on to the findings and recommendations contained in the Commission Staff Report no later than 11/14/12, or 14 days after the date of the filing of the Commission Staff Report; 4. A party’s failure to file written objections to a finding or recommendation shall be deemed as agreement with that finding or recommendation. 5. No later than 11/14/12, or 14 days after the date of the filing of the Commission Staff Report, each party shall notify the Commission whether this matter may be submitted for decision based upon the existing record without hearing. 6. Unless all parties request that this matter be submitted for decision upon the existing record, an informal conference shall be held in this matter on 11/28/12 at 1 :30 at the Commission’s offices. 7. Unless all parties request that this matter be submitted for decision upon the existing record, a formal hearing in this matter shall be held in the Commission’s offices. 8. Each party shall file, no later than 12/7/12, a list of the persons it intends to call as witnesses. 9. Crittenden-Livingston shall publish notice of the hearing in accordance with 807 KAR 5:076, Section 11.
