Final Order Entered: (1) The records of Cases No. 2011-00233 & No. 2011-00390 are incorporated by reference into record of this proceeding. (2) Except where they contradict or conflict with findings contained in this Order, findings contained in Staff's Report are adopted & incorporated by reference as if fully set out herein. (3) Graves District's proposed Phase I rates, set forth in Appendix A, are approved for water service rendered on & after the date of this Order. (4) Graves District's proposed Phase II rates, set forth in Appendix B, are approved for water service rendered on and after January 1, 2014. (5) Within 20 days, Graves District shall file revised tariff sheets containing rates in Appendices A and B. (6) On or about December 1, 2013, Graves District shall notify its customers of Phase II rates by publishing notices in a newspaper of general circulation or placing an insert in bills rendered to its customers. Notice shall be in the form set forth in Appendix C to this Order. (7) Graves District shall remove the effects of the write-down of Utility Plant in Service and Accumulated Depreciation from its books and records that occurred in 2008 and shall restate in its books of original entry. (8) Graves District shall attach to its financial statements, for the year the adjustment ordered above is made, a footnote explaining adjusted balances. (9) As of date of this Order, Graves District shall use whole-life method to calculate annual depreciation expense.

Order Entered: Graves District’s request for a deviation is granted. Graves District is permitted to deviate from 807 KAR 5076, Section 8. Graves District’s application for rate adjustment is considered filed as of the date of this Order. 4. This proceeding is established to review the reasonableness of Graves District’s proposed rates.’offices at 211 Sower Boulevard, Frankfort, Kentucky at a date and time to be announced in a subsequent Order. 11. Each party shall file with the Commission, no later than October 5, 2012, a list of the persons it intends to call as witnesses at the scheduled hearing and a summary of each witness’s testimony. 12. Graves District shall publish notice of the scheduled hearing in accordance with 807 KAR 51076, Section 11, no more than 21 days and no less than seven days before the day of the scheduled hearing. 5. No later than August 30, 2012, Commission Staff shall file with the Commission and serve upon all parties of record a written report containing its findings and recommendations regarding Graves District’s requested rate adjustment. 6. The parties shall file with the Commission their written comments on and any objections to the findings and recommendations contained in the Commission Staff Report no later than September 13, 2012, or 14 days after the date of the filing of the Commission Staff Report, whichever occurs earlier.
