Order Entered: 1. Billy Driskill, Dean McCoy, Steve Mathis, Kenneth Shadowen, and Tim Watkins shall each file with the Commission, no later than April 9, 2018, the original and two copies of the information listed in Appendix A, with a copy to North Marshall Water District. A candidate's failure to file the requested information in a timely manner shall be considered as notice that the candidate does not wish to be considered for appointment to North Marshall Water District's Board of Commissioners. 2. North Marshall Water District shall file with the Commission no later than April 9, 2018, the original and two copies of the information listed in Appendix B. 3. a. Responses to requests for information shall be appropriately bound, tabbed, and indexed and shall include the name of the witness responsible for responding to the questions related to the information provided, with copies to all parties of record and two copies to the Commission. b. Each response shall be answered under oath, or, for representatives of a public or private corporation or a partnership or association or a governmental agency, be accompanied by a signed certification of the preparer or person supervising the preparation of the response on behalf of the entity that the response is true and accurate to the best of the person's knowledge, information, and belief formed after a reasonable inquiry.

Within 20 days of the date of entry of this Order, any person who wishes to nominate a candidate to fill the vacancies on the North Marshall Water District Board of Commissioners shall submit to the Commission in writing the candidate's name, address, and telephone number. 2. Any party filing documents with the Commission shall serve a copy of these documents upon all other parties. 3. Service of any document or pleading shall be made in accordance with 807 KAR 5:001, Section 4(8). 4. Order Entered: 1. This proceeding is initiated to determine whether a vacancy existed on the North Marshall Water District's Board of Commissioners for more than 90 days in 2017. 2. Marshall County Water District is made a party to this proceeding. 3. Any party filing documents with the Commission shall serve a copy of those documents upon all other parties. 4. a. The information requested in Appendix B to this Order is due within 14 days of entry of this Order. Responses to requests for information shall be appropriately bound, tabbed, and indexed and shall include the name of the witness responsible for responding to the questions related to the information provided, with copies to all parties of record, and the original and five copies to the Commission. b.
