Order Entered: 1. South 641 District shall publish, in the newspaper with the largest circulation in the county, a second notice of two vacancies on the South 641 District's Board of Commissioners, attached as Appendix hereto, and supply proof of publication to the Commission within five days of publication. 2. Within 20 days of the date of publication, any person who wishes to apply as a candidate to fill the vacancies on South 641 Water District's Board of Commissioners shall submit to the Commission in writing the candidate's name, address, and telephone number, and reference Case No. 2017-00426. 3. The term of each commissioner appointed shall be four years pursuant to KRS 74.020, and the terms of current commissioners shall be extended to four years and staggered upon the filling of vacancies to be determined upon final order. 4. The Executive Director shall serve a copy of this Order upon the Calloway County Judge/Executive, the members of the Calloway County Fiscal Court, the Calloway County Attorney, and the South 641 District's Board of Commissioners.

Order Entered: 1 . This proceeding is initiated to determine whether a vacancy or vacancies existed on the South 641 Water District's Board of Commissioners for more than 90 days in 2016 and 2017. 2. South 641 Water District is made a party to this proceeding. 3. Any party filing documents with the Commission shall serve a copy of those documents upon all other parties. 4. a. The information requested in Appendix B to this Order is due within 14 days of the date of this Order. Responses to requests for information shall be appropriately bound, tabbed and indexed and shall include the name of the witness responsible for responding to the questions related to the information provided, with copies to all parties of record, and the original and six copies to the Commission. b. Each response shall be answered under oath or, for representatives of a public or private corporation or a partnership or association or a governmental agency, be accompanied by a signed certification of the preparer or the person supervising the preparation of the response on behalf of the entity that the response is true and accurate to the best of that person's knowledge, information and belief formed after a reasonable inquiry. c. A party shall make timely amendment to any prior response if it obtains information which indicates that the response was incorrect when made or, though correct when made, is now incorrect in any material respect.
